Our office hours for scheduling an appointment are Monday – Friday, 9am to 12pm and 1pm to 5pm

No referral is necessary, but if you have an HMO and need an insurance authorization from your primary care doctor, please arrange this before your appointment with us. If you have insurance questions, please call our billing office at (541) 282-6687.
