Insurance
Each insurance company has its own conditions regarding the types of treatments and procedures it covers. It is always advisable to contact your insurance company prior to your visit to discuss your personal insurance coverage and to confirm their acceptance requirements.
Referrals
We will need a referral from your primary care or treating physician to schedule you for an appointment. If your insurance requires authorization, we will also need a referral authorization from your insurance company. We do accept self referrals for hernias and hemorrhoids, but the insurance authorization is still necessary when your insurance company requires one.
It is your responsibility to obtain a valid referral, and it is advisable to confirm that your insurance company accepts that referral before your first visit.

Disability Forms
All disability paperwork needs to be brought to our office during your pre-op visit. Please allow our staff five (5) working days to complete these forms. We will fill out one (1) complementary disability/life insurance type of form. After that, we charge $5 per form. Please make sure to have your portion of the form filled out with your name, birth date and a phone number where we can call you when it is ready to pick up.
