Financial Policies and Payment OptionsSurgery patients have many different medical providers working together to supply your needed services. These include the surgeon, hospital or surgery center, anesthesiologist, and potentially additional providers. In an effort to provide you with the tools you need to navigate the financial aspects of your surgery we have provided the billing contact phone numbers for OSS and our key surgical partners. Billing calls will be returned within 24 hours. We will bill your insurance as a courtesy to you. It is ultimately your responsibility to follow-up with them to make sure your account is paid. We will be happy to bill your secondary insurance carrier if you provide that information at the time of service. We are here to assist you with any billing questions or problems you may have. Please help us by making sure that the reception staff has all of your updated insurance information. You will be required to make your co-pay and/or co-insurance at each visit. For those who have no insurance, you will be expected to pay your balance in full. Our policy is to collect $167.00 as you check in and the remaining balance (if any) as you check out after seeing the provider. Billing statements are mailed monthly. Account balances 90 days past due are considered delinquent. However, we realize that temporary financial problems may affect timely payment of your account. If such problems do arise, please contact us promptly for assistance, otherwise, accounts over 90 days may be placed with our collection agency along with a 30% default fee and/or your care with our practice may be terminated.
Referrals We will need a referral from your primary care or treating physician to schedule you for an appointment. If your insurance requires authorization, we will also need a referral authorization from your insurance company. We do accept self referrals for hernias and hemorrhoids, but the insurance authorization is still necessary when your insurance company requires one. If your insurance company requires a referral to visit our physicians, it is your responsibility to obtain a valid referral through your insurance company. If you do not have a valid referral for your visit you will be responsible for any and all charges incurred at the time of visit. Financial Resources Federal Poverty Guideline Sliding Fee Application For those who may be experiencing financial hardship, sliding fees may be available to private pay patients who have no insurance coverage and have an annual combined household income that falls within the Federal Poverty Guidelines. For the uninsured: Foundation for Health Coverage Education Oregon Department of Human Services links: The Oregon Health Plan (OHP) Oregon Helps Senior Health Insurance Benefits Assistance (SHIBA) Oregon Medical Insurance Pool (OMIP) Oregon Prescription Drug Pool (OPDP) Here in Jackson County, it is estimated that over 33,000 residents qualify for membership in the plan, but only 14% of that number, fewer than 4700 people, have signed up (as of July 2008). Breast and Cervical Cancer Medical Program (BCCM) Transportation assistance for Breast Cancer patients New financial help is available to qualifying (low income) women for assistance with gas, lodging and meals while traveling to obtain a diagnosis or treatment for breast cancer.
