Medical Records
Oregon law allows thirty days for processing all medical record requests. However, it is our goal to have them processed within 7-14 business days.
Requests from legal establishments, life insurance companies, and copying services will be processed with the appropriate signed authorization from the patient, and after the standard fee has been received.
Patients requesting a tranfer of records must submit a written request if the records are being sent to a doctor other than their primary care physician. For your convenience you may either Request Medical Records online or print out and complete a Request for Records Form PDF* .
Please note: we are unable to process requests if the form is not filled out completely.
If you have an urgent request, it is recommended that you call our office at 541-282-6680, and ask to speak to a medical records staff person.
