Referrals to surgeons and our specialty departments can be made by calling

Office hours are Monday – Friday, 9 a.m. to 12 p.m. and 1 p.m. to 5 p.m.

- A convenient Patient Appointment form is included with the Vascular Lab scheduling form PDF*. It is a helpful tool providing your patient with their diagnostic scan appointment time, location information, and directions.
- Indications must be included at the time of scheduling to ensure appropriateness of the ordered exam.
