Vascular Laboratory Scheduling Procedure and Forms
The Oregon Surgical Specialists Vascular Laboratory offers two convenient ways to order a diagnostic scan.
- Download, print, complete the Vascular Lab scheduling Form PDF* and fax to (541) 282-6681.
- Call our office directly at (541) 282-6680.
A convenient Patient Appointment form is included with the Vascular Lab scheduling Form PDF*. It is a helpful tool providing your patient with their diagnostic scan appointment time, location information, and directions.
Please note: indications must be included at the time of scheduling to ensure appropriateness of the ordered exam.
For any special request exams that do not fall within the standard categories, please call our scheduling department at (541) 282-6680.
